The oral bioavailability of CBD is 13 to 19%, while its bioavailability via inhalation is 11 to 45% (mean 31%).[4][5] The elimination half-life of CBD is 18–32 hours.[6] Cannabidiol is metabolized in the liver as well as in the intestines by CYP2C19 and CYP3A4 enzymes, and UGT1A7, UGT1A9, and UGT2B7 isoforms.[2] CBD may have a wide margin in dosing.[16]
Nabiximols (brand name Sativex) is a patented medicine containing CBD and THC in equal proportions. The drug was approved by Health Canada in 2005 for prescription to treat central neuropathic pain in multiple sclerosis, and in 2007 for cancer related pain.[31] In New Zealand, Sativex is "approved for use as an add-on treatment for symptom improvement in people with moderate to severe spasticity due to multiple sclerosis who have not responded adequately to other anti-spasticity medication."[32]
As of April 2019, CBD extracted from marijuana remains a Schedule I Controlled Substance,[13][71][72][73] and is not approved as a prescription drug, dietary supplement, or allowed for interstate commerce in the United States. CBD derived from hemp (with 0.3% THC or lower) was delisted as a federally scheduled substance by the 2018 Farm Bill. FDA regulations still apply: hemp CBD is legal to sell as a cosmetics ingredient, but despite a common misconception, because it is an active ingredient in an FDA-approved drug, cannot be sold under federal law as an ingredient in food, dietary supplements, or animal food.[74] It is a common misconception that the legal ability to sell hemp (which may contain CBD) makes CBD legal.[74]

Preliminary research indicates that cannabidiol may reduce adverse effects of THC, particularly those causing intoxication and sedation, but only at high doses.[18] Safety studies of cannabidiol showed it is well tolerated, but may cause tiredness, diarrhea, or changes in appetite as common adverse effects.[19] Epidiolex documentation lists sleepiness, insomnia and poor quality sleep, decreased appetite, diarrhea, and fatigue.[2]

Cannabidiol can be taken into the body in multiple ways, including by inhalation of cannabis smoke or vapor, as an aerosol spray into the cheek, and by mouth. From 2015 to July 2019, the FDA issued 48 warning letters to 23 American manufacturers of CBD products for false advertising and illegal interstate marketing of CBD as an unapproved drug to treat diseases, such as cancer, osteoarthritis, symptoms of opioid withdrawal, Alzheimer's disease, and pet disorders.[90][91] The FDA said that the letters were issued to enforce action against companies that were deceiving consumers by marketing illegal products for which there was insufficient evidence of safety and efficacy to treat diseases.[90] In July 2019, the FDA stated: "Selling unapproved products with unsubstantiated therapeutic claims — such as claims that CBD products can treat serious diseases and conditions — can put patients and consumers at risk by leading them to put off important medical care. For epilepsy: A prescription cannabidiol product (Epidiolex) has been used. The recommended starting dose is usually 2.5 mg/kg twice daily (5 mg/kg/day). After one week the dose can be increased to 5 mg/kg twice daily (10 mg/kg/day). If the person doesn't respond to this dose, the maximum recommended is 10 mg/kg twice daily (20 mg/kg/day). In some research, higher doses of up to 50 mg/kg daily have been used. There is no strong scientific evidence that nonprescription cannabidiol products are beneficial for epilepsy.
The good news is that numerous states have enacted legislation that allows for the use of CBD. Just four states (Idaho, South Dakota, Nebraska, and Kansas) forbid any access to marijuana. Ten states and Washington, D.C., currently allow for both medical and recreational use of cannabis, including CBD products. The other 36 states allow for the use of medical cannabis in some form, though some of these relegate this to CBD oil only.

It’s complicated. State laws regulating the sale, production and possession of CBD oil widely vary, and many states allow some form of CBD. If passed, new provisions in this year’s Farm Bill would legalize industrial hemp and could make hemp-derived CBD oil legal in all 50 states. But be aware that except for an FDA-approved medication for childhood epilepsy, the Federal Drug Enforcement Administration still maintains that CBD is illegal.
As you may know, industrial hemp and marijuana come from the same genus of flowering plant– cannabis. The term “genus” essentially refers to a sub-family of plants and not a single species. This means that there may be multiple types of the cannabis plant, which are all cannabis but have remarkable differences. So, in terms of scientific classification, multiple species can exist within a single genus, and that’s exactly the case with cannabis.
Cannabidiol has antipsychotic effects. The exact cause for these effects is not clear. But cannabidiol seems to prevent the breakdown of a chemical in the brain that affects pain, mood, and mental function. Preventing the breakdown of this chemical and increasing its levels in the blood seems to reduce psychotic symptoms associated with conditions such as schizophrenia. Cannabidiol might also block some of the psychoactive effects of delta-9-tetrahydrocannabinol (THC). Also, cannabidiol seems to reduce pain and anxiety.
With that being said, one is not necessarily better than the other. CBD can be much more welcoming for those who do not want the potential high that comes with THC. THC may also offer more than just a high, with studies suggesting that it may possess health benefits of its own. More recently, evidence has suggested that THC and CBD can work together through what is known as the “entourage effect”. Taken together, CBD, THC, and the other compounds found in cannabis become more than the sum of their parts, amplifying their effects and working in synergy to support better health and well-being. It’s fine if you want just CBD on its own, but pairing your CBD with some THC may actually be good for you and give you whole plant benefits.
